Alicia” is a 15-year-old female of Puerto Rican heritage. She is the youngest of three children and resides with her biological parents and 17-year-old brother. Her 19-year-old brother is away at college. “Alicia” is in 10th grade and reports doing well academically. “Alice” appears her age and is very pleasant and conversant.
“Alice” presents with signs and symptoms of major depressive disorder and generalized anxiety disorder. She reports feeling overwhelmed by social and academic stressors and struggling to find effective means to manage these feelings. “Alice” denies any substance use history, legal history, or trauma history. She endorses a self-harm history that includes cutting herself on her hips with a razor. This behavior began in 7th grade with infrequent use in recent months that includes several episodes of superficial scratching while hospitalized.
“Alice” has stepped down to the partial hospitalization program from the residential unit where she admitted following her admission to an inpatient unit. She was admitted to the inpatient unit following a report of worsening depression and suicidal ideation with a plan to overdose on her medications, she reports that she never felt that she was going to act on these thoughts. She reports significant social and academic anxiety which has led her to reporting increased hopelessness and some suicidal thoughts due to her feeling that nothing is getting better.
Identify both a Narrative Therapy and CBT intervention, from the scholarly literature, described for use with this client with her presenting problem. Be sure to include specific methods, outcomes reported, and/or evidence referenced when these therapies are used in treatment of the presenting problem. Did the studies you found address cultural diversity and issue of oppression and disadvantage? Were the samples diverse?
Analyze the ethical/social justice related issues you need to be aware of with this client and ways these issues will be addressed (include family, community, and individual level analysis). Consider effects of racial injustice, socioeconomic oppression, homophobia, etc.
Provide a conclusion summarizing your overall impression of practice effectiveness in utilizing one of these approaches. Compare and contrast CBT and narrative approaches as evidence for the therapy approach you would recommend including attention to diversity, and social justice. Include rationale for your recommendation in working with this client and why you believe this intervention is a better fit for the client. You may also include thoughts about integration of these two models.
