Alvin is a 40-year-old male with schizophrenia and lives in supported accommodation with five other residents. He was placed in the supported accommodation in February 2017 when his mother could no longer cope with his anger outbursts and aggression towards her. Mum reported to her son’s Care Coordinator that he was drinking alcohol of up to 12 cans a day and smoking cannabis and using other substances. Mum reported that she saw small syringes and needles in his son’s room and that he was asking her for money each day when he was going out. Mum reported that her son became angry with her and threatened her when she refused to give him money. Alvin denied any knowledge of aggression towards her mum but accused his mum of stealing money from his room. A safeguarding meeting was set up and no evidence was found that mum was stealing money from him, but to prevent harm to mum, who was now elderly, it was deemed that it was unsuitable for Alvin to continue living with his mum.
Alvin has now been living at the supported accommodation for four years and although attempts has been made for him to engage with the substance misuse services, it has been unsuccessful. In February 2019, he visited his General Practitioner (GP) and reported feeling tired, fatigued, felt pains all over his body and that he was feeling feverish most evenings. The GP also noticed that his skin was dry around his hips, heels and at the bottom of his sacrum. He was weighed by his GP and it was noticed that from his last weight taken in December 2018, that he has lost weight from 54.5 kilograms (kg) to 47.8 kg and for his height at five feet four inches, this was a considerable weight loss. However, Alvin reported to his GP that he was eating well and that his appetite was good. The GP discussed investigations to be carried out with Alvin and after discussions he agreed to full blood count (FBC) test, chest X-ray and referral to a dermatologist. The FBC result showed that Alvin’s white blood cells were very low, the X-ray, showed no abnormalities and the dermatologist advised that he uses cetraben emollient cream and this was prescribed. However, with continued weight loss, feeling feverish and little improvement in white blood cells, Alvin was advised to undertake HIV test and he agreed, and results showed that he was positive of HIV and he was started on antiretroviral treatment. He has adhered to his treatment regime throughout, attending for his depot monthly and taking his antiretroviral tablets and warfarin as prescribed.
Alvin had a normal developmental milestone and confirmed having a happy childhood. Alvin’s father is deceased, and his mother is alive. He is the youngest of four brothers and left school at the age of 16 years with no formal qualification and went straight into work as a labourer on a Building Construction site where he started drinking alcohol and smoking cannabis. He has had several relationships lasting for short periods. He reported having a good relationship with his family and confirmed their support for him. He reported seeing his mother daily, because she is elderly, but only sees his brothers when he can.
As a child he witnessed violence in the family and started glue sniffing at the age of 13 years after seeing one of his brothers doing it. He had progressed to using alcohol and smoking cannabis at age 16 years and at the age of 17years, injecting heroin. He used heroin for approximately eight years before he was started on a methadone programme but did not adhere to the programme and disengaged and continued injecting heroin.
He experienced a psychotic episode at the age of 25 years and on assessment, he was noted to be acutely psychotic with severe agitation, auditory and visual hallucinations, and delusional belief. He was admitted to an Adult Acute Mental Health ward. After three weeks on the ward, Alvin was discharged from the ward with a diagnosis of schizophrenia and mental and behavioural disorder due to harmful use of substances.
He became unemployed due to chaotic lifestyle but stayed at home with his mum who became his main carer. However due to increase in use of substances and threatening behaviour towards her, he was placed in a supported accommodation. He visits his care coordinator, a Registered Mental Health Nurse at the GP surgery once monthly for his antipsychotic injection to be administered.
As a result of recurrent DVT (deep vein thrombosis) he is on Warfarin. His card indicates a target INR (international normalised ratio) of 2.5 and dose of warfarin to be prescribed between 3-4.0 mg for the next two months. Alvin has a lump in his left sided groin and has had an MRI (magnetic resonance imaging) of the area but nothing else was found. He was due to attend for a biopsy of the lump, around the end of December last year, but he did not attend for the appointment at his local hospital to undergo a vascular surgery (non-varicose), arranged to improve the circulation in his legs through stenting.
He has been admitted on four other occasions between 2009 to 2014 to Acute Adult Mental Health wards due to relapse in his symptoms of schizophrenia, but has also manged well in the community, which led to a review of his care and treatment to be transferred to his GP.
