Expert answer

The appropriate drug therapy for a patient who presents with MDD and a history of alcohol abuse will depend on the severity of their symptoms. If the patient is experiencing mild to moderate symptoms, they may be treated with antidepressants such as SSRIs or SNRIs. If the patient is experiencing severe symptoms, they may be treated with antipsychotics or mood stabilizers in addition to antidepressants. Patients who have a history of alcohol abuse may be at risk for developing liver damage, so it is important to monitor their liver function closely. The timeframe that the patient should see resolution of symptoms will vary depending on the severity of their symptoms and the medications they are taking. Generally, patients will begin to see improvement within a few weeks of starting medication. However, it may take several months for the full effect of the medication to be felt.
